1. Photograph the issue
Take clear, well-lit photos of the concern from multiple angles so the problem is easy to see.
2. Note the exact location
Include the full street address or cross streets so crews can find it without guessing.
3.Record when you first noticed it
A timeline helps us understand how long the issue has been present and prioritize accordingly. .
4. Write a brief, detailed description
Describe what the problem is, how severe it appears, and any safety concerns it may pose.
5. Include your name, phone number & address
So that we can follow up with you and keep you informed on the status of your report.
6. Send with the right subject line
